Patient Services is a section provided by the Carolina Ear, Nose & Throat Head and Neck Surgery Center to help make your visit to our office a pleasurable experience. This section enhances the patient-physician relationship by giving you 24-hour access to patient specific instructions and communication tools.
New Patients: We are always happy to accept new patients. Please contact our office to schedule an initial consultation. We also suggest that you fill out our on line registration form before your first visit.
Existing Patients: Patients can save time in the office by filling out forms on-line, requesting non-urgent prescription refills, as well as reviewing Pre and Post - Operative instructions.

Please complete the Patient Registration Form before coming in for your appointment. If any patient information has changed since your last visit, for example, your insurance carrier or your address, please complete another Patient Registration Form as well.
If you are a new patient or if you have not been to our office in 2 years, please fill out the New Patient and Long Term Follow-up Patient Information form in addition to the Patient Registration form.
If you are having a minor child brought to the office, and the parent or legal gaurdian will not be present, please fill out the Authorization for Consent to Medical Treatment for a Minor.

What is a Patient Advocate?
Patient advocates support and promote patients' rights as they navigate the health care system.
Whether or not you are ill, managing your health can be confusing and overwhelming. And when things go wrong, it can be hard to get your voice heard by the health care professionals around you. Health care providers and administrators do not intend to make things difficult for patients, but the system can be so complicated that compassion and understanding get lost.
Lynne, our patient advocate can guide you through the confusing maze of health care with caring and sensitivity. She can focus exclusively on your needs and help you resolve concerns about the quality of your care, get the care you need, and ensure that your voice is heard and that you are included in decision-making.
